  • “Planning for the Future Checklist”

    “Planning for the Future Checklist”

     

    The Planning for the Future Checklist” was developed and supported through a partnership between the United Way of Allegheny County’s 21 and Able Initiative (http://www.21andable.org/) , the Pennsylvania Bureau of Special Education, and the Pennsylvania Training and Technical Assistance Network, with input from stakeholders across Pennsylvania.

    The “Checklist” is a roadmap for youth with disabilities 14 to 21 year’s old who are transitioning from high school to continue their education, to work, live independently, and participate actively in their communities to the greatest extent possible.

    The “Checklist” was designed as a resource for youth, parents, and educators to guide their conversations about secondary transition before, during, and after annual IEP meetings and/or agency meetings.

    The “Checklist” addresses the areas of education after high school, employment, and community living, that parents and youth, who have exited secondary schools, believe these are critical issues in preparing students with disabilities for expectations of life after school.
