Placement Requirements
Written placement request from the university or college must include:
- Description of the assignment
- Certificate level
- Certificate subject
- Assignment start and end date
- Student's profile, bio and resume
- Employee Health Record Form
- Act 34 - PA Criminal History Clearance
- Act 114 - FBI History-Service Code 1KG6XN
- Act 151 - Child Abuse Clearance
All clearances must be within 1 year of starting date
Physical must be within 1 year of placement
TB test results must be within 90 days of placement
